Physician practices in the region that are skilled in preventive care and chronic disease management are being recognized by an insurer.
Priority Health, a Grand Rapids-based health plan, announced last week its 2015 Quality Award recipients, which recognized approximately 130 network physician and health care practices in Allegan, Kent, Muskegon, and Ottawa counties for their performance in delivering preventive care and chronic disease management to patients.
The program
The Quality Awards honor the top performing physicians and health care practices achieving the highest overall scores in preventive care and chronic disease management quality measures evaluated through the Priority Health Primary Care Provider Incentive Program.
More than 80 physician practices in Kent County alone were recognized for their performance in 2014; while three practices in Allegan, 19 in Muskegon and 25 in Ottawa also picked up awards.
Each health practice was selected after analyzing the results of the incentive program, which benchmarks preventive and chronic management measures against national standards and evidence-based medicine.
Priority Health said the physician practices received nearly $22 million in incentive pay in 2015 as a result of their performance in 2014.
The performance
Priority recognized almost 250 practices statewide, including 130 in the four-county region, for meeting or exceeding the national 90th percentile in clinical quality measures for breast cancer screening, cervical cancer screening, childhood immunizations, asthma control, LDL cholesterol control for diabetes and high blood pressure control.
Michael P. Freed, president and CEO, said Priority Health has partnered with primary care physicians across Michigan to improve the quality, access and affordability of care for members for nearly 20 years.
“It’s proven time and time again that the greatest health care outcomes are achieved when patients, health plans and physicians work together,” said Freed. “We’re proud of the results we are seeing today.”
In 2014, nearly 70 percent of Priority Health group plan members with high blood pressure had their chronic condition under control, which is above the national average of 63 percent, according Priority Health’s website. Priority Health network physicians also provided routine childhood immunizations to nearly 87 percent of patients in 2014, which is above the national average for group health plans at 76 percent.
The impact
Priority Health has partnered with its network physicians and practices to improve health care outcomes, identify and treat potential health problems in a cost-effective manner, and avoid the incidence of chronic disease complications.
The Quality Awards program was established in 2003 with the recognition of only 30 practices providing exceptional care for patients. Priority has awarded more than $198 million to practices in support of primary care over and above the standard payment of services.
